I spent nearly two hours working with a technician from Microsoft, but he was unable even to determine why this was happening. I sent the manuscript to Janelle, and she spent considerable time just finding the problem: whole chapters had footnotes that were coded as regular text rather than footnotes. She was able to find and correct this even though both Amazon and Microsoft had been unable to do so. And her final invoice was exceedingly reasonable given her efforts.
