I have a roofing company.  I want an interested customer to be able to enter their information into a form and have it generate a contract that the customer can initial, sign and submit online instantly.  

There is a service qktract that I like, which allows you to put a custom contract on your website. is the demo video.  

The problem is, I don't want my competition to see the name of the service i used to create the contract because they will join the service and copy me, so I want to "duplicate" this service with no branding.  I think it would be better if this all happens within my website (wordpress), and not through an external link as qktract does.

I would also like for the information submitted to automatically be able to be sent into Vtiger, Zoho CRM, and other various systems that we may utilize in the future. (Not a complete deal-breaker, but I would like that a lot)

I want this to look nice, and inviting, to encourage customers to sign the contract online instead of having to send salespeople to the homes to get contacts signed.

The style can be changed completely to look extra friendly, or a slideshow style, so that a few questions get answered at a time, then it slides to the next questions or group of questions.
