How frequently do patients receive care from outside their state of residence in a given year?

We research about Health care utilization by consumers in the United States.  Looking for a reliable set of information that would describe in a given year what percent of the population had at least on health care encounter in a state other then the one they resided in at the start of the year.  

There are several obvious sources including:
- Relocation of a person from their place of residence on January 1 of a given to another state within that year.
- Residents of Medical Service areas that saddle the boundary between two states
- Referral to centers of excellence in one state to another
- Referral of under-served populations to other states such as Alaskans to the Continental US

It is possible that an interstate network that Healthplans refer to when their members receive care outside of the Insurers local network (within state of residence).  

The research needed would provide an estimate as to the percent of American's that at any one point in a given year receive care outside of the state in which they resided at the start of the year.  

I am interested in hearing proposals from individuals familiar with the domain that are available to work on this immediately and provide a results of 4 hours effort as quickly as possible.
