I am looking for a simplistic record keeping system windows based software. It will be used to expedite the creation of medical patient records. Every record created with the software should be stored locally, and only be deleted by the Administrator. Record files need to be only opened by the software.
Upon installation, the software will ask for an Admin password with all privileges (create, edit, and delete records) and a user password with only create privileges.
The software should have the ability to create new patient and look up existing patients visits/records.
Once a new visit record is chosen, the patient records form should pop up. It will be divided into 5 sections (patient info, patient health, etc). In each section, there will be subsections such as wound history, allergies, medications, and, etc with options that consist of either dropdowns, checkboxes, and free text lines. If an option in a subsection is filled out, that subsection will duplicated to the output file, if no options in a subsections are checked, the subsection will not be placed on the output file.
All the sections, subsections options will be provided on start of contract.
For example if under the subsection of Medications, there will be the following options:
0 Antibiotics: 0 Steroids: 0 Other: _________
If Antibiotics and Other will be checked, and filled out, in the output file it should show Medications: The patient is on Antibiotics ....., and Other ..... .
Every field in every subsection should have the option of text being added to it. If you double click on any field a small textbox should appear.
After everything is filled out and the export button is clicked, the visit record should be saved locally and the output file should be generated in .doc or pdf for easy printing.
There are a few minor additional details that I did not include that will be discussed during the interview process.
Time frame for this project is 2 Weeks for completion and testing.
