You will need to perform a search to find all of the hospitals in a given US State, which will be designated to you upon hiring. The following link can be used to find a thorough list of the hospitals in each state. http://en.wikipedia.org/wiki/List_of_hospitals_in_the_United_States The data is to be transferred to an excel spreadsheet that is ready to be imported into ACT. The order of the cells in excel is below. In the US, not all hospitals have Emergency Rooms, so you will have to define Y or N for each Hospital. You should be able to find lists of Trauma centers through simple searches on the internet. Every Trauma Center in the United States has a trauma number 1,2,3,4 or 5. Not all hospitals have a Trauma designation. You can generally find this with a Google search or on the hospital’s website.
This will be the format:
Suite or Unit Numbers
Phone Number (if there is an extension it is ext. 0000 or whatever the extension is)
Emergency Room Y/N
