Looking for a developer that can create an short online assessment. The assessment will comprise of 8 simple questions/statements that the user will score themselves on. On completion of the questions the scores will be plotted onto a circumplex, and will indicate areas they may want to focus on for improvement. Assessment will be hosted on website homepage, need to be branded and would also be great if a more detailed report/version could be emailed to the user. I have an example of what I would like to create it would be great if people could share via social media, download to pdf, print option but all completely branded.

I need line drawings to show how a medical catheter is adjusted and inserted into a patient. The total number of individual drawings needed is of the order of 10. They must be simple, unambiguous etc and involve both upper torso outlines and more detailed close ups of particular features of the catheter. The size of the illustrations will be of the order of 3cm x 3cm. I would prefer to work with somebody who has previous experience of medical illustrations. I have attached a file with similar types of illustration.

We need a set of illustrations to help teach/guide construction workers to follow proper procedures for handling and installing of specific materials. Primarily a set of DOs & DON'Ts to help workers understand what to do and what not to do when working with these materials. Line drawings would probably work best. We look for a freelance Instructional Designer with specific experience in Medicine and/or Biology or Pharmacology. Both senior and junior profiles will be evaluated.
