I'm thinking to create the docs using Typeform.com
Basically, in the hiring process, we have the caregiver read all the terms and conditions, job position, a quick assessment, and background check form, etc.
This is how our hiring process goes:
1. Fill out skill assessment
2. Read, initial, and sign all the onboarding paperwork (apprx. 9 pages)
3. Fill out the Home care registration form to sign up as home care aid
4. Print the livescan form and complete
I want to streamline this process where everything is on the ipad. For Example, the basic information will be filled out first, and then a checklist can be made for the caregiver to fill out, and then when asked for the resume, prompt the careigver to take picture, then when needing the drivers license, take picture, cpr/first aid, picture, etc.
Then everything is embedding into this one document.
After the documents are all signed, an emailed copy will be sent to caregiver, and us (the agency).
I am not expecting you to make the user interface from scratch, but I need you to put it together in a format using either an App or something like www.typeform.com and re-create the forms on that website to be uploaded on ipad.
The point is, I want to be able to pull up a blank doc, and hand it to the employee to type in and fill out all the info. But i want it to be easy to use, minimalistic, and simplified where it doesn't take the person an hour to fill out.
Open to suggestions.
