Survey Creation Directions and Process
• Create two surveys on survey monkey (see sample link below)
• Each survey serves as an opportunity for teachers to provide feedback on one WEEK of trainings
• Use a format similar to this survey
• Keep questions 1 and 2 the same as the ones on this link
• For question three, instead of the sessions that are included in the attached survey, use the sessions that are listed below instead. For academic content sessions that are department specific (i.e. science vs ela, etc – please specify)
• Use the questions below for questions 4 to 7 (this survey won’t have a question 8)
o 4) Which session(s) from this week do you feel will have the biggest impact on your ability to effectively teach successful during the school year? Why?
o 5) Which session(s) do you feel will NOT have a big impact on your ability to teach successfully during the school year? What could’ve been shifted about the session to accomplish that goal?
o 6) Please provide any feedback on the structure and flow of the last week. What was effective? What wasn’t?
o 7) Please share any additional comments or feedback to support ongoing reflection and refinement. What should we keep doing? What was missing from the first week of AO? Feel free to comment on communication, logistics, content or process
