CPA Letter Notorized

Summary

I need a letter from a CPA, on letter head and notorized possibly apostilled, stating my intent to file self employment taxes as seen in the attachment below, it can be almost identical. I just need it notorized and done asap. my accountant is currently on vacation and im trying get this done by august 12th. Please let me know if you can help. It is for spanish digital nomad visa requirements.
