Care Coordination Specialist

We're hiring a Registered Nurse for a full-time, immediate-start freelance role supporting an ACO (Accountable Care Organization) partnership working with skilled nursing facilities. This is ongoing, full-time work (30+ hrs/week) for an RN who wants steady, structured remote hours with real responsibility from day one. What you'll be doing: Reviewing and verifying facility census data for accuracy Coordinating patient information across care transitions Identifying and logging payer source details Completing and submitting required forms and documentation What we're looking for: Active RN license required Experience in SNF, ACO, or care coordination settings preferred Highly detail-oriented and comfortable with accuracy-driven work Available to start immediately and commit to full-time hours Comfortable working independently with minimal oversight
