Comfort Letter from Accountant

Please include your cost for producing this letter to be considered. I would like a comfort letter/verification from an accountant. The letter will need to have my passport number, confirmation that I own my business, my role as the business owner, that I render services for my business remotely, and verification of my income. 
I will provide a general template for you to adapt as you see fit. I can provide the articles of organization for the business with my name on them, personal and business tax returns from last year, invoices and bank deposits showing active billed and received amounts. This needs to be from a legitimate accounting business/service and written on a proper business letterhead. This does not need to be from a CPA. Any accountant with a legitimate U.S. business is fine.
