Looking For Graphic Designer to Create Street Sheets Design (Pocket-Sized Resource Guide)

We're looking for a designer to help us create street sheets for a local health clinic client. The design must match the client's brand guidelines (we will provide a brand guide upon hiring along with additional information) and print specs. Each item in the list includes the Organization Name, Primary Category, Secondary Categories, Address, Primary Contact Phone, Email (optional, not on ever location), Website, Hours of Operation, and a Brief Description. Please provide relevant examples demonstrating your ability to adhere to established brand guidelines as well as any print work you've done that may be similar to what we're looking to accomplish with the street sheets.
