Medical Document Review & Clinical Records Review

We are seeking a qualified medical professional with appropriate credentials and experience in medical records review to assist with the organization and professional review of existing healthcare documentation. Responsibilities may include: • Reviewing existing medical records, invoices, statements, and related documentation for organization and completeness • Creating chronological summaries of existing medical documentation • Identifying missing records or documents that may need to be requested from healthcare facilities • Helping organize documentation received from hospitals, physicians, laboratories, and other healthcare providers • Reviewing medical terminology and documentation for clarity and consistency • Identifying questions or areas that may require clarification from the treating provider • Assisting with administrative preparation of records and documentation for the client or their authorized representative • Providing general professional observations regarding the organization and contents of existing records, within the freelancer’s lawful scope of practice Candidates should have relevant U.S. healthcare credentials and/or professional experience in medical records, clinical documentation, medical coding, healthcare administration, or a related field. Licensed medical professionals are encouraged to apply where their credentials are relevant to the work. All patient information must be handled confidentially and only with appropriate authorization. Work must comply with applicable privacy laws, professional licensing requirements, and Upwork’s Terms of Service. Candidates must perform the work themselves and may not misrepresent credentials, sign documents on behalf of another professional, or provide services outside their authorized scope of practice.
