Seeking a licensed psychiatric provider — MD, DO, or PMHNP — to establish care via telehealth: an initial evaluation, ongoing monthly visits, and completion of standard employer leave-of-absence certification paperwork. Scope of engagement: Initial telehealth evaluation (video, 45–60 min) — full psychiatric intake. Established mental health history and a recent significant life event. Completion of a standard Healthcare Provider Certification form (FMLA-style) for a medical leave of absence - diagnosis, functional limitations, expected duration with a defined initial period and reassessment date, treatment plan. There is a firm 15-day submission window, so the intake needs to happen this week or early next. Monthly follow-up visits for the duration of the leave (est. 3–6 months) — continuing treatment, plus 30-day recertification forms if requested by the leave administrator. Requirements: Active, unrestricted US license (psychiatry or psychiatric-mental health NP). Arizona licensure or AZ telehealth registration preferred — patient is located in Arizona. License and NPI verified before engagement. Prior experience completing FMLA, short-term disability, or employer leave certifications. Same-week intake availability.
