Summary

SENIOR HEALTHCARE SOFTWARE DEVELOPER OR TEAM NEEDED TO ADD AVAILITY, COMPREHEND, DOCTRONIC AND VITL RX STYLE FEATURES INTO EXISTING EHR Timeline: 30 to 60 days Existing EHR is already built We already own and operate a functioning EHR and telehealth platform. I am looking for a highly experienced healthcare software developer, architect, or small development team to take our existing EHR and add several major enterprise capabilities into it. The four companies below are the closest references for what I want: 1. Availity https://www.availity.com/ 2. Comprehend Health https://comprehendhealth.ai/ 3. Doctronic https://www.doctronic.ai/ 4. VITL Rx https://vitlrx.com/ The goal is to take the major capabilities from these platforms and build them directly into or deeply integrate them with our existing EHR. I want one unified system and one unified experience. I do NOT want four separate portals loosely connected together. I want these features to feel native to our EHR. I also do NOT want someone rebuilding technology that already exists if there is a reliable API, SDK, white label product, licensed platform, existing codebase, open source framework, or proven integration that can get us there faster and for less money. If you have already built something very similar, that is exactly what I am looking for. If you already have code for major parts of this that you can legally reuse and customize, even better. The end result needs to be enterprise ready, multi tenant, white label, and something we can resell to other clinics, physician groups, MSOs, telehealth companies, and healthcare organizations. PART 1 AVAILITY STYLE INSURANCE, ELIGIBILITY, PRIOR AUTHORIZATION AND RCM FEATURES Reference: https://www.availity.com/ https://www.availity.com/api-marketplace/ I want insurance functionality available directly inside our EHR. The goal is to reduce the need for clinic staff to constantly log into payer websites or separate clearinghouse portals. I am open to using Availity itself, another clearinghouse, another API vendor, or a combination of vendors. I want the developer to recommend the fastest and smartest approach. REAL TIME ELIGIBILITY AND BENEFITS From the patient chart, staff should be able to run an insurance eligibility check. The response should be translated into a clean and easy to understand interface. Information should include where available: Insurance carrier Plan name Member ID Group number Subscriber Relationship to subscriber Active or inactive coverage Effective date Termination date Plan type Individual deductible Family deductible Deductible met Deductible remaining Copay Coinsurance Individual out of pocket maximum Family out of pocket maximum Amount met Amount remaining Primary care benefits Specialist benefits Urgent care benefits Telehealth benefits Behavioral health benefits Prescription benefits when available Service specific benefits In network status Out of network status Patient responsibility Benefit limitations Coverage restrictions I also want: Real time eligibility checks Batch eligibility Automatic eligibility before appointments Automatic rechecking when appropriate Alerts for inactive coverage Eligibility history stored in the patient chart Ability to save eligibility results Ability to associate eligibility results with an encounter Human readable benefit summaries Access to the raw payer response when needed PATIENT COST ESTIMATION Where payer data supports it, I want: Expected allowed amount Estimated deductible responsibility Estimated copay Estimated coinsurance Estimated patient responsibility Estimated payer responsibility Estimated total out of pocket cost PRIOR AUTHORIZATION I want prior authorization workflows available directly inside the EHR. The user should be able to select: Patient Payer Service CPT or HCPCS code Diagnosis Provider Facility The system should determine whether prior authorization is required whenever the payer or API supports it. The user should then be able to: Create the authorization Submit the authorization Add diagnosis information Add procedure or service information Add provider information Add facility information Upload clinical notes Upload supporting documentation Upload medical records Add attachments Add comments Track the authorization Possible authorization statuses should include: Draft Submitted Pending Additional information required Approved Partially approved Denied Cancelled Expired The system should store: Authorization number Approved units Approved dates Start date End date Payer comments Denial reason Required follow up PRIOR AUTHORIZATION WORK QUEUE I want a work queue showing: New requests Pending requests Needs documentation Needs follow up Approved Denied Expiring soon CLAIM STATUS I want claim information visible directly inside our EHR. The system should display where available: Submitted Received by payer Accepted Pending Processing Rejected Denied Paid Partially paid Patient responsibility Additional information required Claim number Payer claim ID Submitted amount Allowed amount Paid amount Deductible amount Copay amount Coinsurance amount Adjustment amount Patient responsibility Remark codes Denial codes Denial descriptions Service dates Service lines CLAIM ATTACHMENTS Where supported, staff should be able to submit: Medical records Clinical notes Supporting documentation Requested claim attachments CLEARINGHOUSE AND CLAIMS If practical through the clearinghouse or API strategy, I want the EHR to support: 837P professional claims Claim creation Claim validation Claim scrubbing Claim submission Rejections Corrections Resubmissions 276 and 277 claim status 835 ERA Payment posting Denials Appeals Patient balance tracking We do NOT need to build a clearinghouse from scratch. I want to integrate one. INSURANCE DASHBOARD I eventually want one clean insurance and RCM dashboard showing: Eligibility checks Patients with inactive coverage Prior authorization requests Authorizations pending Authorizations approved Authorizations denied Claims submitted Claims pending Claims rejected Claims denied Claims paid Outstanding accounts receivable Expected reimbursement Collected reimbursement Denial rate Payer performance PART 2 COMPREHEND HEALTH STYLE AI SCRIBE AND CLINICAL AI Reference: https://comprehendhealth.ai/ The AI scribe must live inside our existing EHR. The provider should not have to leave our platform to use it. AMBIENT AI SCRIBE During an appointment, the provider should be able to start recording. The AI should listen to the clinical conversation and generate structured documentation when the visit is complete. The provider should also be able to: Record a live visit Dictate after the appointment Give a verbal summary Upload audio Generate a note from a transcript Generate a note from existing patient data AUTOMATIC NOTE CREATION I want support for configurable note types such as: SOAP note History and physical Progress note Consult note Follow up note Telehealth note Primary care note Urgent care note Weight management note GLP 1 note TRT note HRT note Mental health note Sexual health note Dermatology note Peptide therapy note Lab review note Medication follow up note Custom organization templates The AI should identify and populate information such as: Chief complaint History of present illness Medical history Surgical history Family history Social history Allergies Current medications Review of systems Objective findings Assessment Diagnosis Treatment plan Orders Medication changes Follow up Patient education DIRECT EHR FIELD POPULATION This is important. I do not want the AI to simply generate one large paragraph. Where technically practical, the AI should populate structured fields inside the EHR. The provider should be able to review, edit, approve, and sign. PROVIDER SPECIFIC AI LEARNING I want the AI to adapt to the way each provider documents. For example, one provider may prefer short notes while another provider prefers more detail. The system should support: Provider specific templates Organization specific templates Specialty templates Shared templates Custom instructions Preferred terminology Preferred note length Preferred formatting Learning from provider edits where appropriate AI DOCUMENTATION COMPLIANCE REVIEW Before a note is signed, I want the AI to review the documentation for: Completeness Missing sections Conflicting information Medical necessity concerns Documentation weaknesses Potential payer issues Audit exposure Missing objective information Missing treatment rationale Coding and documentation mismatch I want the ability to create organization specific compliance rules. Example: A TRT follow up note must document certain required elements. If something is missing, the provider should receive a warning before signing. AI CODING ASSISTANCE Based on the documentation, the AI should be able to suggest: ICD 10 codes CPT codes Modifiers where appropriate Supporting documentation requirements Potential coding conflicts Missing documentation The provider or billing team should always be able to review before anything is finalized. AI CHAT INSIDE THE PATIENT CHART Inside a patient chart, the provider should be able to ask questions such as: Summarize this patient's medical history What happened during the last visit Summarize the last five visits What medications are they currently taking What medications have they previously taken What allergies do they have What were their most recent labs How have their lab values changed What diagnoses are documented What treatments have already been tried Draft a referral letter Draft a letter of medical necessity Draft a prior authorization Draft an appeal Draft patient instructions Draft a work note Summarize this entire chart Suggest billing codes based on the documentation The AI needs to securely use the data already available in the patient's chart. MOBILE SCRIBE If practical, providers should be able to use the scribe on: Desktop Tablet Mobile phone The provider should be able to record on one device and access the completed note inside the EHR. PART 3 DOCTRONIC STYLE AI PATIENT CARE, REFILLS, APPOINTMENTS AND PROVIDER ESCALATION Reference: https://www.doctronic.ai/ This is one of the highest priority pieces. I want patients to be able to interact with an intelligent AI healthcare assistant before a human provider becomes involved. AI PATIENT CHAT A patient should be able to open our website, app, or patient portal and simply type what is wrong. Examples: I think I have a UTI I have had a sore throat for three days I need my blood pressure medication refilled The AI should begin a natural conversational intake. I do NOT want a long static questionnaire. The questions should change dynamically based on the patient's previous answers. AI SYMPTOM INTAKE The AI should collect relevant information such as: Symptoms Duration Severity Location Onset Relevant history Medical conditions Previous episodes Current medications Allergies Treatments already attempted Pregnancy possibility when relevant Contraindications Red flags Emergency symptoms The system should dynamically determine which questions are necessary. SAFETY AND TRIAGE The system should identify situations that require escalation. Possible outcomes may include: Emergency room 911 Urgent in person care Same day provider review Video appointment Specialist referral Routine async provider review Safety rules need to be configurable. AI GENERATED CLINICAL SUMMARY When intake is complete, the AI should generate a structured summary including: Patient complaint Relevant history Symptoms Pertinent positives Pertinent negatives Red flags Current medications Allergies Relevant medical history Clinical summary The provider should see this directly inside our EHR. AI TO HUMAN PROVIDER HANDOFF The patient should not have to repeat everything when the case is escalated. The provider should receive the complete AI conversation and structured summary. The provider should then be able to: Review asynchronously Ask another question Start secure chat Start video Schedule an appointment Create a treatment plan Order labs Write a prescription Refer the patient Decline treatment Escalate care PRESCRIPTION REFILL AUTOMATION I want a sophisticated prescription refill workflow. The patient requests a refill. The system should collect: Medication Dose Frequency Prescriber Reason for medication How long the patient has taken it Last dose Side effects Current symptoms Relevant vitals when required Relevant labs when required Contraindications Medication specific safety questions The system should use information already available in the chart so we are not repeatedly asking the patient for information we already have. The AI should prepare the refill encounter for provider review. The provider should be able to: Approve Modify Deny Request additional information Require labs Require a video appointment Require an in person appointment If approved, the prescription should move into our prescribing and pharmacy workflow. CONDITION SPECIFIC AI FLOWS I want configurable AI pathways for different services and conditions. Examples: Primary care Urgent care UTI Sinus infection Upper respiratory infection Pink eye Rash Cold and flu symptoms Allergies Minor infections Weight management GLP 1 intake GLP 1 follow up Dose escalation Side effects Refills Weight tracking TRT HRT ED Hair loss Lab review Men's health Women's health Mental health intake Mental health follow up Medication follow up The system should make it possible to add more workflows over time without rebuilding the core AI engine. APPOINTMENT SCHEDULING If the AI determines that the patient needs a provider, the system should immediately offer available appointments. The patient should be able to see: Real time provider availability Appointment type Specialty Patient state Provider license Service type Time zone Video availability Soonest available appointment The patient should be able to book without leaving the workflow. SMART PROVIDER MATCHING The system should route patients based on: Patient state Provider licenses Provider availability Specialty Service line Clinic Organization Appointment type Provider capacity The system should be able to identify the next available qualified provider. VIDEO ESCALATION The patient should be able to move directly from AI intake into a video visit. The provider should already have access to: The full AI conversation Structured intake Existing patient history Medication history Previous visits Relevant labs No duplicate intake. LONGITUDINAL AI HEALTH RECORD The AI should intelligently use information already in our EHR. Future conversations should use relevant information such as: Conditions Allergies Medications Previous encounters Previous AI conversations Labs Prescriptions Treatment plans PATIENT AI AVAILABLE 24 HOURS A DAY Long term, I want the AI to become the first interface for many patient needs. Patients should be able to use it for: Symptoms Healthcare questions Appointment navigation Prescription refill requests Existing treatment questions Preparing for an appointment Understanding provider instructions Understanding diagnoses Understanding lab results Finding the correct type of care Requesting referrals Follow up questions All with appropriate safety controls and provider escalation. PATIENT HEALTH COMMAND CENTER The patient portal should bring together: AI conversations Medical record Appointments Previous consultations Prescriptions Refill requests Medication list Treatment plans Labs Provider communication This should all use the same data already stored in our EHR. PART 4 VITL RX STYLE MULTI PHARMACY MARKETPLACE Reference: https://vitlrx.com/ I want a multi pharmacy marketplace built directly into our existing EHR. The goal is to give providers one interface where they can access many pharmacies instead of constantly switching between different pharmacy portals. PHARMACY NETWORK We eventually want to connect: 503A compounding pharmacies 503B outsourcing facilities Specialty pharmacies Other appropriate pharmacy partners The initial target could be 20 or more pharmacies, with the ability to expand significantly over time. UNIVERSAL MEDICATION SEARCH The provider should be able to search a medication inside our EHR. Examples: Tirzepatide Testosterone Cypionate BPC 157 The system should search the catalogs of connected pharmacies. SIDE BY SIDE COMPARISON The provider should be able to compare pharmacy options based on: Pharmacy Medication Formulation Concentration Strength Dosage form Quantity Size Price Shipping cost Fulfillment time State availability Patient specific availability Clinic stock availability where appropriate Current availability Pharmacy type The provider should be able to filter and sort results. CURRENT PRICING Where pharmacy integrations allow, I want: Current pricing Contract pricing Organization pricing Special pricing Shipping price Total cost Clinic cost where applicable PATIENT COST where applicable BEST PRICE AND FASTEST FULFILLMENT The system should be able to identify: Lowest price Fastest fulfillment Preferred pharmacy Recommended option based on configurable rules PRESCRIBE OR ORDER FROM ONE SCREEN From our EHR, the provider should be able to: Open the patient Select medication Compare pharmacies Select a pharmacy Select product Enter SIG Select quantity Add refills where appropriate Submit The provider should not need to open several pharmacy websites. MULTIPLE MEDICATIONS AND BATCH WORKFLOWS The system should allow efficient ordering of multiple medications when clinically appropriate. PATIENT SPECIFIC AND CLINIC STOCK WORKFLOWS Where legally and operationally appropriate, the system should support: Patient specific prescriptions Clinic stock ordering 503A workflows 503B workflows The platform should know what type of transaction is permitted for each pharmacy and product. PRESCRIPTION AND ORDER STATUS I want one dashboard showing statuses such as: Submitted Received by pharmacy Processing Needs clarification Pharmacist review Compounding Ready Shipped Delivered Cancelled Rejected SHIPMENT TRACKING The patient and clinic should be able to view: Carrier Tracking number Ship date Estimated delivery date Delivery status The system should support automated: SMS notifications Email notifications Patient portal notifications PATIENT MEDICATION INSTRUCTIONS Patients should be able to see: Medication Dose Administration instructions Provider instructions Relevant educational information COMPLETE ORDER HISTORY The provider should be able to see: All orders All pharmacies Order status Cost Medication Patient Date Tracking information Pharmacy Prescriber PHARMACY ADMIN AND INTEGRATION LAYER I want a scalable way to onboard and manage pharmacy integrations. Different pharmacy connections may use: API SFTP Webhook ePrescribing infrastructure Direct integrations Other secure integration methods Each pharmacy profile should support information such as: Pharmacy name States served Licensure 503A or 503B Product catalog Available formulations Pricing Shipping Fulfillment rules Availability API credentials Integration settings MULTI CLINIC MANAGEMENT Organizations should be able to configure: Preferred pharmacies Approved medications Preferred products Location specific rules Pricing Ordering rules Pharmacy routing rules Organizations should also be able to view: Purchasing behavior Order behavior Pharmacy performance Fulfillment times Medication utilization PHARMACY ANALYTICS I eventually want reporting on: Prescription volume Orders by pharmacy Orders by medication Orders by clinic Orders by provider Average fulfillment time Average delivery time Order failures Pharmacy issues Pricing Patient utilization ENTERPRISE MARKETPLACE ARCHITECTURE The architecture should support different commercial models such as: SaaS fees Platform fees Transaction fees Enterprise contracts Clinic subscriptions Other legally permissible arrangements The technology should be flexible enough to change business and compliance rules without rebuilding the entire system. MOST IMPORTANT REQUIREMENT EVERYTHING MUST LIVE INSIDE OUR EXISTING EHR I do NOT want: Our EHR plus a separate AI portal plus a separate insurance portal plus a separate pharmacy portal plus a separate scheduling system. I want one system. From the same patient chart, a provider should eventually be able to: View AI patient intake Use the AI scribe Ask AI questions about the chart Run compliance checks Verify insurance View benefits Check prior authorization Submit prior authorization Check claim status Review the patient Conduct an async encounter Start a video visit Create orders Search medications Compare pharmacies Prescribe Track pharmacy orders Schedule follow up Message the patient ENTERPRISE AND WHITE LABEL REQUIREMENTS This is critical. Our existing EHR serves multiple healthcare businesses. Every new feature needs to work with our existing multi tenant architecture and be something we can resell. We want to offer white label versions to: Physicians Medical groups Telehealth companies MSOs Medical practices Healthcare entrepreneurs Provider organizations Each organization may have its own: Branding Domain Providers Patients Pharmacy relationships Insurance relationships AI workflows Clinical protocols Templates Pricing Permissions Feature access FEATURE TOGGLES As the platform owner, we need to be able to turn features on and off by organization. For example: Clinic A AI Scribe on AI Patient Intake on Insurance off Pharmacy Marketplace on Clinic B AI Scribe on Insurance on Pharmacy Marketplace on AI Patient Care on This allows us to create different enterprise packages. USAGE METERING Where practical, I want usage tracking for: AI scribe minutes AI notes AI patient conversations Eligibility transactions Prior authorization transactions Claim transactions Prescriptions Pharmacy orders Video visits API calls This is important because we intend to sell these features as enterprise services. WHITE LABEL EXPERIENCE Where vendor agreements permit, our customers should see our platform or their own brand rather than the underlying vendor. I want the experience to feel completely integrated. DO NOT REBUILD TECHNOLOGY THAT CAN BE PLUGGED IN This is extremely important. I have no interest in paying someone to unnecessarily recreate mature technology. If Availity APIs can provide a feature, integrate them. If another clearinghouse has a better API, recommend it. If an AI scribe SDK can provide most of the Comprehend functionality, use it. If you already built an ambient clinical scribe, show me. If you already built a conversational medical intake system, show me. If you already built prescription refill automation, show me. If you already built a pharmacy marketplace, show me. If existing pharmacy infrastructure can accelerate the project, use it. I want our development budget spent on: Integration Orchestration Workflow User experience Enterprise architecture White labeling Differentiation I do not want the budget wasted reinventing commodity infrastructure. BUDGET Approximately $8,000. I understand that building Availity, Comprehend, Doctronic, and VITL Rx completely from scratch would cost dramatically more than this. That is not what I am asking for. I am specifically looking for someone who has already built similar healthcare technology and knows how to combine: Existing APIs Existing infrastructure Existing code Healthcare vendors Licensed technology Open source technology where appropriate White label components Strategic custom development Third party licensing and API costs can be identified separately. TIMELINE 30 to 60 days. I want aggressive implementation. I am not looking for a long discovery project. I am not looking for a demo. I am not looking for a proof of concept. I want the requested capabilities integrated into the existing EHR and operational. ONLY APPLY IF YOU HAVE BUILT SOMETHING VERY SIMILAR Please do not apply simply because you are a strong general developer. I specifically want people with experience in: EHR and EMR Telehealth Healthcare SaaS AI healthcare Clinical AI Ambient scribing Insurance eligibility RCM Claims Clearinghouses Prior authorization ePrescribing Compounding pharmacy technology Pharmacy marketplaces FHIR HL7 X12 HIPAA Multi tenant healthcare software Enterprise white label SaaS The ideal applicant reads this post and says: I have already built most of this. IF YOU HAVE BUILT SOMETHING SIMILAR, SHOW ME I want to see: Working products URLs Screenshots Videos Live demos Previous integrations Architecture API integrations Existing modules If you have built something comparable to Availity, I want to see it. If you have built something comparable to Comprehend Health, I want to see it. If you have built something comparable to Doctronic, I want to see it. If you have built something comparable to VITL Rx, I definitely want to see it. REQUIRED APPLICATION QUESTIONS Do not send me a generic cover letter.
