Web App for Retirement Scorecard

I am looking for an experienced web developer to build a simple internal application for a financial advisory firm. An advisor will enter prospect information and complete a retirement-planning questionnaire. The application will use scoring rules that we provide to automatically generate a polished, branded, one-page retirement scorecard. The first version should include: A clean, easy-to-use data-entry form Yes/no, multiple-choice, and text-entry fields Automatic scoring based on predefined rules Red, yellow, and green status indicators A live preview of the completed scorecard A professionally designed 8.5 × 11-inch portrait layout One-click PDF generation and download A reset button for starting a new prospect Required-field validation No permanent storage of prospect information in the initial version We will provide the scorecard categories, questions, scoring logic, branding, colors, logo, and a visual example of the desired one-page report. The application should be easy for a nontechnical employee to use. It should also be straightforward for a future developer to update questions, scoring rules, colors, and wording. This first phase does not require user accounts, a database, CRM integration, payment processing, or financial-planning software integration.
