Zoho Forms & Workflow Builder Needed

We are seeking an experienced **Zoho Forms / Zoho workflow specialist** to convert our existing DocuSign documents and paper-based workflows into professional, automated Zoho forms. We are a behavioral health organization and need someone who understands how to build forms that are secure, easy to use, and properly routed after submission. ## Project Includes The builder will convert and automate documents such as: * Client intake packets * Adult and minor intake forms * Referral forms * Consent forms * Release of Information forms * Treatment-related forms * Assessment and treatment plan signature forms * Staff/internal forms * Other existing DocuSign documents ## Required Functionality Forms must include: * Conditional logic for adults vs. minors * Required parent/guardian information and signatures when applicable * Electronic signatures * Required-field validation * Auto-population of repeated information such as: * Client name * Date of birth * Guardian name * Intake date * Other information already entered earlier in the form * Automatic date population where appropriate * Staff-only sections * Ability for staff to complete portions of a form after the client/referral source submits it * Email notifications * Automated routing based on form responses * Referral status workflows * Ability to notify referral sources of status changes * Internal staff assignments and notifications * Document/PDF generation after completion * Organized storage of completed forms * Reporting and tracking capabilities * Integration with other Zoho applications when appropriate ## Referral Workflow Example A referral source submits a referral. The referral should automatically route to designated office staff. Staff reviews the referral and verifies insurance/eligibility. Based on the result, the system should automatically: * Update the referral status * Notify the appropriate staff * Notify the referral source when appropriate * Record relevant dates * Track whether the referral moved forward or was declined * Maintain a referral tracking database/dashboard We do **not** want someone simply recreating PDFs online. We need someone who can analyze the current workflow and build the automation, routing, conditional logic, and reporting behind the forms. ## Experience Required Please apply only if you have strong experience with: * Zoho Forms * Zoho CRM and/or Zoho Creator * Zoho Flow * Workflow automation * Conditional form logic * Electronic signatures * Form-to-PDF/document generation * Automated email routing * Healthcare or behavioral-health workflows preferred * HIPAA-sensitive workflows strongly preferred ## When Applying Please provide: 1.
