Senior Laravel + Vue Engineer — Refactor of a Health Platform

We have developed a platform for healthcare management. This engagement is a refactor and modernization of an existing system, not a greenfield build. The domain, the database and the screens already exist and are in production. Your job is to restructure the codebase behind them into something maintainable, correctly multi-tenant, well tested and ready for the next two years of feature work — without regressing behaviour that clinics rely on every day. We are looking for one experienced engineer who can own this end to end and work independently against clearly defined milestones. The platform is organized into domain modules. The refactor covers the following areas, in roughly this order: • Multi-tenancy and data scoping — a headquarters must read data across all of its locations and be able to write into any one of them, while a single location may only ever read and write its own. This rule must hold uniformly across every module rather than being re-implemented per screen. Revenue cycle — invoicing, payments, claims, insurance eligibility verification, benefit summaries, prior authorizations, coordination of benefits, payer enrollment. • Scheduling — appointment calendar, provider availability, operatory assignment, conflict detection, recalls and reminders. • Communications — SMS, email and voice channels, unified inbox, per-patient communication history, campaigns. • Growth — leads pipeline, patient segmentation, feedback surveys, treatment acceptance tracking. • Inventory — supplies, medications, warehouses, stock movements, purchase orders, vendors. Required Experience • 4+ years of production Laravel, including queues, jobs, policies and Eloquent at scale • Strong Vue 3 with the Composition API and TypeScript in strict mode • Demonstrable experience refactoring a large existing codebase, not only building new features • Comfortable writing tests first and treating a green CI pipeline as part of the deliverable • Able to work independently, ask precise questions and flag risks early • Clear written English and overlap of at least 4 hours with our working day • Available for roughly full-time work across the 8 weeks — this timeline does not suit a few hours a week Nice to Have • Inertia.js, Filament, Laravel Octane or Horizon experience • Healthcare domain experience (claims, eligibility, EDI) • Multi-tenant SaaS architecture experience
