Care Coordinator or case management

Care Coordinator We are seeking a compassionate, detail-oriented Care Coordinator to provide care coordination services for medically complex and/or mentally fragile clients while serving underserved populations within the community. The Care Coordinator is responsible for managing client records, conducting comprehensive assessments, coordinating care with healthcare providers, and ensuring timely, accurate documentation. This role includes establishing primary care providers, scheduling specialty appointments, arranging transportation, connecting clients with community resources, and assisting with applications for public benefits such as SSI/SSDI, SNAP (food assistance), and cash assistance programs. The Care Coordinator also helps clients access housing resources, including rental assistance programs, and advocates for clients to ensure they receive appropriate medical, behavioral health, and social services. The ideal candidate will have experience in a healthcare or human services setting, such as a Medical Assistant, LVN/LPN, Registered Nurse (RN), Social Worker, Case Manager, or a related field. Candidates should possess excellent communication, organizational, and problem-solving skills, with the ability to work collaboratively with clients, families, healthcare providers, and community organizations. Experience working with vulnerable or underserved populations is highly preferred.
