Convert PDF to InDesign File

Summary

I need someone with Adobe InDesign experience to convert a long, basic designed PDF into an editable InDesign file. The task requires mostly copy and pasting from the PDF, which is a long document, but very basic in design. The fonts and accuracy of the design do not need to be exact. The final deliverable will be an Indesign file with the content from the PDF inserted so that it appears the same as the PDF. Sample File Attached
