Convert Two Existing Assessments into SurveyMonkey

Project Overview: I have two completed assessments Stress Load Audit Cognitive Load Audit The surveys are already fully designed with all questions, categories and rating scales completed. I need an experienced SurveyMonkey user to transfer them into SurveyMonkey and apply my branding. No survey design, copywriting, scoring model development, or psychometric work is required. Scope of Work Survey Setup Create both surveys in SurveyMonkey. Copy and paste all questions and categories from my existing documents. Set up pages/sections as required. Configure a 1-5 rating scale throughout. Test functionality and user experience. Branding Add logo. Apply brand colours. Create a clean, professional appearance consistent with my coaching and consulting business. Participant Information Include the following questions at the start of each survey: Name Email Address Age Group Gender Industry Job Title / Profession Years of Experience Organisation Name (optional) Reporting Requirements The assessments are not based on a total score. All responses should be captured and reported. Reporting should allow me to: View all participant responses. Identify questions rated 4 or 5. See trends by category. Export results to Excel/CSV. Generate basic visual reports/charts. Compare results by: Industry Gender Age Group Profession/Job Title Years of Experience Coaching Focus While all responses are recorded, coaching discussions focus primarily on responses rated: 4 = Often 5 = Always Reports should make these responses easy to identify.
