We need an Excel workbook that automatically fills a laboratory order form by placing "X" marks in the correct checkbox locations while preserving the original A4 PDF layout exactly as provided. The project is strictly related to Excel form automation and print layout alignment. The PDF form contains no patient data and will be provided for template/layout purposes only. Requirements Sheet 1 – Test Selection - Create a clean table listing all tests exactly as they appear on the form. - Include a selection column where tests can be marked using either: - "X" - Dropdown list (Blank / X) - Compatible with Excel 2019 and newer versions. - No macros required unless absolutely necessary. Sheet 2 – Auto-Filled Original Form - Convert the first page of the provided PDF into a high-quality PNG image. - Place the form image in Excel while maintaining exact A4 dimensions (21 × 29.7 cm). - The form must look identical to the original document. - When a test is selected in Sheet 1, an "X" should automatically appear in the corresponding checkbox location. - Use formula-based logic whenever possible, for example: =IF(Selection!C2="X","X","") - Printing output must align perfectly with the original form. Sheet 3 – Summary - Automatically display a list of all selected tests. - Optional: Include a count of selected tests. Technical Requirements - Must work in Excel 2019 or newer (Windows). - No external plugins or add-ins. - Clean workbook structure and organized sheets. - Precise positioning and alignment for accurate printing. Deliverables - Fully functional Excel workbook. - Original form accurately aligned to A4 size. - Automatic checkbox marking based on selections. - Clean and organized worksheet structure. - Brief instructions explaining how to edit or add tests in the future.
