Create 2 editable files

I am looking for 4 different editable 8.5 x 11" files. I will require full access to the editable files. File 1: I am looking for a baby daily log that includes feeding times, diaper change time and whether it was wet/dirty, sleep/naps with a "from _ to _" and space for medications, notes, activities, and overall mood of the child. I would like this log in 2 different formats. 1 format in a table format and the other in a more visually appealing format. File 2: I am looking for a medication tracking log. It should include the date, medication, dose, time and notes. I would also like the medication tracking log to be in a seperate format that seperates into 4 sections: "morning medication", dose, time, Sunday - Saturday." "afternoon medication, dose, time, Sunday - Saturday." "evening medication, dose, time, Sunday - Saturday." and "other medication", dose, time, Sunday - Saturday.
