Fillout Form Builder — Conditional Logic Assessment

I'm looking for an experienced Fillout specialist to build a guided identity assessment form for a personal development program. What needs to be built: A multi-section assessment with advanced conditional logic. The form has 4 sections, each following the same 7-step flow. Based on the participant's selection in each section, only the relevant checklist is shown — all others are hidden. The form also includes text-based reflection exercises at the end. Key elements: 4 sections with conditional logic (single-select triggers different checklists) Multi-select checklists (some appearing twice as separate fields) Short-text and long-text reflection fields Read-only profile pages displayed conditionally Progress indicator, mobile-responsive, clean premium design What is NOT part of this project: No GoHighLevel, no webhook, no integrations of any kind Your deliverable is the live form + a complete field map (Google Sheet listing every field, its type, and section) Source materials provided: full content document with all checklist items, profile texts, and instructions.
