Health Screening Microsoft PowerApps Build

We are a healthcare services company migrating off Excel onto the Microsoft Power Platform. We have a complete, detailed functional specification ready to share. This is a well-defined build, not a vague "figure it out for us" project. What we need built: A Dataverse data model (Members, Outreach Events, Staff, Regions, Import Batches) with proper relationships and rollup fields A Canvas Power App with two role-based interfaces: a fast, simple logging screen for frontline staff (must log an entry in under 15 seconds) and a supervisor screen for bulk member import, assignment, and roster management Conditional workflow logic (certain fields appear only after specific outcomes are selected) A Power BI dashboard with defined KPIs (reach rate, conversion rates, productivity by employee and region, daily/weekly views) One-time Excel data migration of current records plus a repeatable import template This is a HIPAA environment. Work will occur within our compliant M365 tenant with a signed BAA (we handle licensing on our side). You must be comfortable building to that constraint.
