Looking for an experienced 3D modeler to create a 3D printing file from a flat image design

We are a small jewelry and sculpture shop that has recently taken a commission to produce a 1ft by 2ft commemorative bronze plaque. The model will be 3d printed, dipped in wax, and cast into bronze at a local foundry. Our expertise is in hand carving in wax, so we're looking for 3D modeling support. We have a two dimensional design that uses photographs and text. The main image is a portrait, but there are also birds and a figure. It's important that the portrait is a good likeness, and that the bird species is accurate. The portrait subject is wearing glasses, so experience with rendering glasses into bas relief is desired. We're looking for someone who is comfortable with highly detailed 3D modeling, and is familiar with preparing files to be 3D printed. I'm asking for proposals, as I'm unsure what a budget should be for this project. Thank you!
