Medical Writer – Patient Rehab Reports (Senior Care / Assisted Living)

Overview We are seeking a highly detail-oriented medical writer to assist with the preparation of Residential Habilitation reports for senior citizens participating in assisted living and long-term care programs. This is not a content writing position. We are looking for someone who can review medical records, diagnoses, behavioral concerns, and functional limitations, then develop comprehensive clinical narratives that clearly explain why a resident requires additional one-on-one support services. The ideal candidate has experience with medical writing, nursing documentation, rehabilitation reports, care planning, social work documentation, case management, gerontology, psychology, behavioral health, or related healthcare fields. What You'll Be Writing You will primarily complete the following report sections: 1. Detailed Summary of Needs 2. Detailed Weekly Schedule 3. Detailed Care Plan Goals Occasionally, there may be an additional form requiring basic completion. Templates and examples will be provided. Scope of Work For each resident, you will receive: * Medical diagnoses * Medication lists * Physician documentation * Behavioral concerns * Functional limitations * Incident history (falls, wandering, aggression, etc.) * Existing care plans and supporting documentation Using this information, you will create detailed reports that typically range from 5–10+ pages in length. What We Need We need a writer who can tell the resident's story from a clinical perspective. The reports must clearly explain: * How each diagnosis affects the resident's daily functioning * The risks associated with the resident's medical conditions * How cognitive impairments impact safety and independence * Why routine assisted living services are insufficient * Why additional one-on-one support is medically necessary * What negative outcomes may occur without intervention Examples include residents with: * Dementia * Alzheimer's Disease * Parkinson's Disease * Schizophrenia * Bipolar Disorder * Anxiety Disorders * Depression * Stroke History * Traumatic Brain Injury * Fall Risk * Wandering Behaviors * Aggression * Elopement Risk * Cognitive Impairment * Poor Safety Awareness You should be comfortable discussing how these conditions increase the likelihood of: * Falls * Injury * Hospitalization * Medication non-compliance * Wandering and elopement * Behavioral escalation * Social isolation * Decline in activities of daily living * Increased caregiver burden * Deterioration in overall health and quality of life Example Rather than simply writing: "Resident has dementia and requires supervision." We need detailed clinical explanations such as: "The resident's dementia significantly impairs short-term memory, judgment, and safety awareness. Due to cognitive decline, the resident frequently forgets staff instructions, becomes disoriented within familiar environments, and requires repeated redirection throughout the day. Without consistent one-on-one intervention, the resident is at increased risk for wandering, falls, medication non-compliance, and injury. Additional individualized support is necessary to maintain safety, reinforce routines, and prevent further functional decline." Qualifications Preferred experience in one or more of the following: * Medical writing * Nursing * Case management * Social work * Occupational therapy * Physical therapy * Behavioral health * Psychology * Gerontology * Long-term care * Assisted living * Skilled nursing Strong English writing skills are required. Experience reviewing medical records and translating clinical information into detailed narratives is highly preferred.
