Healthcare Intake Form & Patient Automation Specialist Needed (Jotform/Google Forms + Google Sheets) Project Overview I own a growing outpatient psychiatric practice in California, VICKY LOLO, A Professional Nursing Corporation, and I am looking for an experienced freelancer to build a professional digital wellness screening and patient intake system that I can use at community wellness events, health fairs, and networking events. My goal is to create a seamless experience where attendees can scan a QR code, complete a mental health screening on their phone, and have their information automatically organized for follow-up and appointment scheduling.
