Need Help Fixing a HubSpot Workflow

Summary

I've been setting up a few things in HubSpot myself, but I've hit a point where one of my workflows just isn't doing what I need it to do. The goal is simple. When someone fills out one of our forms, I want HubSpot to update the contact, send a follow-up email, and notify our team. I've spent more time on it than I wanted to, so I'd rather have someone who knows HubSpot take a look and get it working properly. If you've built HubSpot workflows before, this should be a fairly quick task. If everything goes smoothly, I have a few other things I'd like help with afterward, including cleaning up our CRM and setting up a couple more automations.
