Clinical Care Coordinator — Hepatology Quality Improvement (Part-Time, Remote) Job type: Part-time contract · 10–20 hrs/week · US only About the project Halo Solutions partners with hepatology at a Boston academic medical center on a clinician-led quality improvement program to close evidence-based care gaps for patients with primary biliary cholangitis (PBC). We are hiring a clinical coordinator to support outreach, tracking, and workflow execution — under hepatology supervision. This is an operations and care-coordination role. You will not make independent treatment decisions. What you'll do - Contact patients identified through our safety-net workflow who may benefit from follow-up or monitoring per guideline-based criteria - Review electronic medical records to assess software-identified care gaps - Help coordinate hepatology follow-up, track outreach attempts, and document closure status in our workflow tools - Coordinate with existing hepatology navigators and clinic staff - Maintain structured weekly status on open gaps (aggregate metrics) - Follow all HIPAA and site compliance requirements What we're looking for - 2+ years in care coordination, patient navigation, or clinical ops in GI/hepatology, chronic disease, or specialty clinic settings - Experience with electronic medical records, especially Epic - Experience with phone outreach, recall programs, or gap-closure workflows - Clear written communication and reliable follow-through on SOPs - Comfortable working remotely with secure tools; health system onboarding required before PHI access Nice to have - Bilingual (English and Spanish) - Familiarity with cholestatic liver disease or rare disease programs - Prior work with academic medical centers or multi-site health systems - Experience supporting QI or registry-based programs
