Software Engineer (Healthcare / Digital Health Platform) Location: Remote (U.S.-based preferred) Engagement Type: Fractional / Contract (20–40 hours/week, flexible) About Crosswalk Health Crosswalk Health is a virtual-first specialty care company focused on improving outcomes for patients with chronic respiratory disease, including COPD. We partner with leading primary care organizations, accountable care organizations (ACOs), and Medicare-focused health systems to deliver high-quality, guideline-based pulmonary care. Our model combines in-person diagnostic testing, pulmonologist-led interpretation, asynchronous peer consultations, and virtual patient care—powered by a technology platform designed to scale across geographies and clinical settings. We are rapidly expanding across multiple states and are building a modern, secure, and interoperable technology stack to support our growth. The Role We are seeking high-performing software engineers to support key initiatives across our platform. This role is ideal for engineers who enjoy ownership, solving complex real-world healthcare problems, and working in fast-paced, mission-driven environments. You will contribute to critical projects such as: Integration with electronic health record (EHR) systems Enhancements to our Ruby on Rails-based web applications Medical device data integration (e.g., spirometry and diagnostic tools) Platform scalability, reliability, and performance improvements Security, compliance, and data infrastructure enhancements This is a hands-on role with meaningful impact on clinical care delivery and operational efficiency. Key Responsibilities Design, build, and maintain scalable backend services and APIs Develop and enhance features within a Ruby on Rails web application Lead or support integrations with EHR systems (e.g., via HL7, FHIR APIs) Integrate and normalize data from medical devices and third-party vendors Collaborate with clinical, operations, and product teams to translate workflows into technical solutions Improve system performance, observability, and reliability Implement and maintain security best practices aligned with HIPAA and SOC 2 requirements Contribute to architectural decisions and technical roadmap Preferred Qualifications We are looking for candidates who reflect the profile of engineers at market-leading digital health and specialty care platforms: Technical Expertise Strong experience with Ruby on Rails and modern web frameworks Strong experience with Python and backend software / data engineering Experience building and scaling API-driven architectures Familiarity with cloud environments (AWS preferred) and infrastructure-as-code Experience with data pipelines, ETL processes, and structured/unstructured healthcare data Knowledge of interoperability standards (HL7, FHIR, CCD) Experience integrating with EHR systems (e.g., Epic, Cerner) Familiarity with medical device integrations or IoT data ingestion is a strong plus Security & Compliance Experience working in HIPAA-compliant environments Familiarity with SOC 2 controls, secure coding practices, and data privacy principles Understanding of authentication/authorization frameworks (OAuth2, JWT, SSO) Product & Execution Mindset Proven ability to work in early-stage or high-growth environments Strong ownership mentality with the ability to operate independently Experience translating clinical or operational workflows into scalable technical solutions Comfort working cross-functionally with clinicians, operators, and non-technical stakeholders Collaboration & Communication Excellent written and verbal communication skills Ability to document systems, decisions, and tradeoffs clearly Experience working in fully remote, distributed teams Nice to Have Experience in digital health, virtual care, or value-based care models (e.g., ACOs, Medicare Advantage) Familiarity with healthcare analytics, risk adjustment, or population health data Exposure to AI/ML applications in healthcare workflows Experience supporting multi-state clinical operations or logistics platforms Why Join Crosswalk Health?
