Senior Laravel Developer - Long Term Maintenance of Hospital Management System

We run the backend of a hospital management system (HMS) for a private hospital: a Laravel 10 / PHP 8.1 REST API covering patients, surgeries, inpatient stays, emergency, laboratory (with lab-device integration), radiology, pharmacy, blood bank, NICU, inventory, billing, dual-currency accounting, HR/payroll and reporting. We are not rebuilding anything. We want an experienced developer who is comfortable working inside a large existing codebase, for a long-term maintenance relationship. THE CODEBASE - Laravel 10, PHP 8.1, MySQL, Docker Compose + Nginx - ~100 controllers, ~97 Eloquent models, 264 migrations - JWT authentication (tymon/jwt-auth), roles/permissions (spatie/laravel-permission) - Model audit trail (owen-it/laravel-auditing), Excel import/export (maatwebsite/excel) - Well documented: a full system-features document plus a professional security and business-flow audit with a prioritized fix list. Both will be shared with the hired candidate under NDA. THE WORK - Phase 1 — paid trial (fixed price): 2–3 small, well-defined fixes from our audit's priority list (authorization middleware, a config hardening change, a database unique-constraint migration). This is how we evaluate each other. - Phase 2 — ongoing retainer (est. 15–30 hrs/month to start): work through the rest of the audit list, bug fixes, small features, dependency/security updates, occasional code review. - Phase 3 — larger projects quoted separately as they come up (e.g. a data-type migration across financial columns). NICE TO HAVE - Healthcare / hospital-information-system / LIS-integration experience - Arabic language WORKING EXPECTATIONS - Ideally a long-term relationship (12+ months) — the system will always need ongoing maintenance. - The budget set on this project is the monthly retainer for the maintenance contract; additional hours/work can be discussed at additional cost. - Acknowledge normal requests within 1 business day; occasional urgent fixes need faster turnaround (exact terms agreed together). - The person who applies must be the person doing the work (no agency hand-offs).
