We are seeking a dedicated Home Health Care Coordinator to manage client care plans and schedules during evening hours. The ideal candidate will have experience in home health care and be able to communicate effectively with clients and caregivers. Responsibilities include coordinating care schedules, managing client records, and ensuring smooth operations. This is a part-time role with a focus on evening shifts. You will work 1pm-8pm EST.
