Healthcare Workflow Automation Architect (Jotform, CRM & process Automation)

We are a healthcare startup looking to automate our patient intake process and build the foundation for a scalable workflow system. We currently have a basic patient intake form built in Jotform and are looking for an experienced workflow automation specialist—not just a form builder—to redesign and automate the process. Project Scope: Build advanced conditional logic and dynamic branching within Jotform. Integrate Jotform with our CRM to eliminate manual data entry. Design an organized, scalable workflow that can be expanded as our business grows. Document the workflow for future enhancements. This is the first phase of a larger initiative to automate the patient journey from lead intake through diagnosis, scheduling, treatment, and follow-up. The ideal candidate has experience with workflow automation, CRM integrations, Jotform, and platforms such as Make.com, Zapier, or similar tools. Healthcare experience is a plus.
