NOTE: Any interested vendors must send a portfolio of samples of online surveys (and accompanying personal reports based on a person's answers) they created for other companies. INFO ON THIS PROJECT I work with the National Association of Evangelicals. We are working on a project to create self-assessment surveys and individual reports to assist pastors who are struggling with their personal and church finances. To do this, we are going to create 10 training modules. Each training module will begin with a 25-40 question online self-assessment survey. Similar in look/feel/design of http://livingforwardassessment.com/home/assessment.html (except we will have people answer on a 1-4 scale instead of 1-10 scale. Attached is a sample list of 40 question for 1 of the Modules. After someone takes the survey, we would like them to receive a customized report based on their answers. We also want to tabulate their "score" (total points and % based on the maximum number). See the bottom of the attached excel spreadsheet. PLEASE BE SURE TO SEND US SAMPLES OF SURVEYS WITH INDIVIDUAL REPORTING RESULTS you've created for others. The project will take about 3 months to complete. There will be total of 10 modules with their individual self-assessment surveys. Each survey will have 25-40 questions. We would like these surveys and reports built in a commonly known and used programming language. After the 3 months, we would like to be able to be trained enough to be able to update questions on our own and not have to rely on ongoing support.

Overview Build a website based on suggested design and functionality we supply to you. The website will serve as a fully functional, simple e-commerce site that we will use with a small customer base (less than 50 people) for the launch of our start-up. If you need more information before committing to your price, I’m happy to provide it. General Scope of Work 1. The site will match families with babysitters and will be similar in functionality to www.sittercity.com and www.care.com but with a few key differences. This site will be MUCH simpler and will only serve one city. 2. The site will have roughly 15 – 20 pages to start, including 3 “sign-up” forms and a payment checkout page for parents who want to become members and contact a sitter. 3. Families and sitters will input basic information into a form. This will allow families to post jobs, and it will allow sitters to create a profile page. (Again, similar in functionality to www.sittercity.com and www.care.com, but much simpler.) 4. Families will be able to search for sitters closest to them based on the addresses entered on a form. Clicking on the page will allow the family to see the individual profile page for the sitter. 5. At launch, sitters won’t be able to search for jobs closest to them based on the addresses entered on page. However, this functionality will need to be built with this initial job because we’ll launch that functionality soon after initial launch. When the page is live, clicking on the job will allow the sitter to see the family’s profile as well as the job. 6. At launch, sitters WILL be set up to receive an e-mail notification any time a new job is posted. 7. We will also need a mechanism to track donations made by us and families. One element of the checkout process will allow families to “pay it forward” for charity, and we’ll match whatever they give. 8. We will also need a mechanism to track a prize for our sitters. Each time they take a job that is verified by a parent, they increase their chances of winning a cash prize. Each time they refer a friend to the site (referral counts when we approve the new profile of referred person), they also increase their chances of winning and we add another $5 to the prize total. 9. We will also need a mechanism to allow parents to refer other parents. They will win something when they refer other parents. 10. We will need two almost identical versions of the homepage. One will be the main home page leading out to the rest of the site. The other will be a Nashville-specific home page leading out to the rest of the site minus the general homepage. 11. We also need a relatively robust admin back end and database that will allow us to easily see and record how customers are interacting with the site.

We are a non-profit start up working with the urban and underserved youth of our city. I have purchased the domain names through GoDaddy and have already purchased a year of hosting through WP Engine. Originally I was looking to create this site on my own, but the more I look at things, the more i am feeling out of my depths. So i figured i would check to see how much it would cost to contract a professional to design/create a website on our behalf. We have spent a lot of time looking at the different available themes ... We really like the flow, features, and lay out of Outreach Pro theme found on the Studio/Word Press site. however, we are open to whatever suggestions you have. I have been told that the Outreach Pro theme is very basic and looks generic and blah blah blah, i don't know. • Our logo is black and white (attached above). So in terms of color schemes ... We want to preferably stay within the scope of greyscale look (blacks, whites, charcoals, grey, etc.). • We want a site that is able to be smoothly navigated on a variety of platforms (desktop, mobile, tablet, etc.). • Something that clearly conveys our message, and encourages a strong call to action (online giving and signing up for our mail list). • Something that is clean and simple yet looks professional and high end. Looking for a developer to create a few pages that will work to upload a csv, then also make the data in the database downloadable to a spreadsheet. The upload with save data to the database and then display the data on a page like an index in a table and also output an html file for each data row. The html file will have a design like a website but would display the data in a way that looks good. I will supply the html template. I know nothing about web development, but I do know the solar installation business and I currently employ 64 people. I'll need someone who is capable of taking this project from start to finish. I am looking to hire someone to develop a website that is only one page, but then starts the user into an application process that takes them through a series of steps; where as they answer the questions and potentially upload files, it is changing the value of what we are going to charge to install the solar project. It would be referencing a database of values. These values are always changing in this industry and we would need the option to change them regularly. At the end of the application, the website would spit out a cost to install solar. I will also want SEO on this project. Our goal is to use this site to empower the customer to fill out as much data as possible, which would be doing "work" that has value to reduce the cost of solar as well as educate our customers, thus creating the best solar installation experience possible. This is a goal to help bring solar ownership to the people at the lowest cost possible and keep solar out of the hands of multi-billion dollar corporate conglomerates.
