This covers approximately 16 months of activity across multiple providers and specialties. This engagement has two phases: Phase 1 is a defined audit and reconciliation project to get all claims current. Phase 2 is an ongoing monthly retainer to manage new claims, EOBs, and reimbursements on a rolling basis going forward. We are looking for one specialist to handle both phases and build familiarity with our accounts over time. Scope of Work 1. Document Gathering & Organization • Pull all EOBs from Aetna member portal (access will be provided) for both covered individuals, February 2025 to present • Pull any available EOBs or correspondence from RHS • Collect and organize provider bills and statements provided by client • Create a master claims tracker spreadsheet organized by date of service, provider, amount billed, amount paid, and balance 2. Claims Audit & Reconciliation • Cross-reference all Aetna EOBs against provider bills to identify discrepancies, unpaid balances, or overpayments • Identify all claims where Aetna has processed but RHS secondary claim was never submitted • Flag any claims approaching or past RHS timely filing deadlines (typically 12 months from date of service) • Identify any out-of-pocket payments made by client that may be eligible for reimbursement 3. Secondary Claims Submission (RHS) • Prepare and submit outstanding claims to RHS with Aetna EOBs attached • Prioritize claims at risk of timely filing expiration • Follow up on any submitted claims that require additional documentation • Document all submissions with dates, reference numbers, and expected timelines 4. Reimbursement Processing • Identify all claims where client is owed reimbursement (paid out-of-pocket, secondary not yet applied) • Submit reimbursement requests as applicable • Provide client with clear summary of expected reimbursements and timelines 5. Final Deliverable • Clean master spreadsheet with all claims, statuses, amounts, and action items • Summary memo outlining: what was submitted, what is pending, what was recovered, and any unresolvable items with explanation • Recommendations for maintaining clean COB going forward Phase 2: Ongoing Monthly Claims Management Following completion of the initial audit, we are seeking continued monthly support to keep claims current and ensure no reimbursements are missed going forward. Monthly Responsibilities • Review and track all new EOBs from Aetna as they are issued • Submit new claims to RHS secondary within timely filing windows • Follow up on any pending or denied claims from prior months • Reconcile provider bills against EOBs and flag any discrepancies • Track and submit reimbursement requests for any out-of-pocket payments • Provide a brief monthly summary of claims processed, amounts recovered, and any outstanding items Estimated Monthly Volume Two covered individuals, multiple providers and specialties. Exact monthly volume will vary but is expected to be light-to-moderate — this is personal (not business) insurance management. Ideal Candidate • Experience with coordination of benefits (COB) between primary and secondary insurance payors • Familiarity with Aetna EOB formats and claims submission processes • Experience navigating employer group health plans and self-funded plan submissions • Detail-oriented with strong spreadsheet/tracking skills • Able to handle sensitive personal and financial information with discretion • US-based strongly preferred for data privacy and compliance reasons • Prior experience with insurance reimbursement audits or medical billing reconciliation required What Will Be Provided • Access to Aetna member portal (shared securely) • RHS plan documents and member ID information • Physical and/or scanned provider bills and statements • Any correspondence or prior EOBs already on hand • Prompt responses to questions and quick turnaround on any additional documents needed Note: A confidentiality/NDA agreement will be required prior to sharing any account access or personal documents.
