You send the information you'd like entered into a google form. Please include the google email account details (including password) that will ultimately manage this form and its compiled information.
Create Google Form
I'll put the information you've requested into the Google Form and submit it for your review. Depending on the tier you purchased, I will make changes per your edit suggestions.
