You will receive an HTML file with Javascript and CSS files for formatting and validation, with indications on where to include the file in your existing HTML template. I provide one free revision to ensure that there are no clashes with existing javascript or page styles.
I can also help you configure the server to store the data passed from the form and send out an auto-responder for example.
No frameworks or additional code is required for the form to work.
