For example, a certain scene is too detailed, the dialogue feels bland and unrealistic, the character's actions don't match the impression the writer has initially given, plot holes, inconsistencies etc.
I take a break before I come back to the story the second time. This time, I give it a more thorough read where I actively look for things that are off, amiss, or need improvement. Once I'm finished with the second reading, I check my comments to make sure I didn't make a mistake or get something wrong. There's nothing worse than providing inaccurate and half-committed feedback.
