I have been transcribing for five years now - my average speed is 2 hours 30 minutes for an hour of audio. Once I've transcribed a file, I go through it at least twice to ensure that there are no grammatical errors or omissions. Upon completion thereof, I use a template of my own design (or one provided by my client) and carefully format the transcribed, proof-read file before returning it to the client.
