Define the issue
You select one repeat incident pattern to focus on. Examples: lockout not followed, bypassing guarding, powered industrial truck contact, hand injuries
Diagnose the system
I review the materials that shape the work: recent incidents and near misses, the written procedure, training materials, and relevant checklists or forms. I conduct 2–3 stakeholder interviews to understand how work is done under real conditions.
